Social Health Connect (SHC) is for adults who are over 18 years old, living in the Caboolture and Kilcoy regions, and who have unmet social needs negatively impacting on their health and wellbeing.

SHC strives to build meaningful relationships and build community. Supporting social health can positively impact your physical, mental and emotional health and wellbeing.

  • Develop person-centred goal plans to address loneliness and social isolation
  • Explore local groups and social activities/opportunities people might be interested in
  • Build capacity and address barriers that prevent social inclusion.

What does this program offer?

Person-centred, strengths-based approach using a Social Prescribing model. SHC has an open referral pathway but strongly encourages referrals from GP’s and health professionals. The team link clients into groups and social opportunities that are meaningful for them and work with clients around capacity building to address barriers to participation.

Our team members are called Link Workers, and they are skilled at building relationships, are well connected to Caboolture and Kilcoy and have the knowledge to navigate both government systems and community services.
